Rwanda has taken a bold leap toward equitable universal health coverage by expanding Mutuelle de Santé, the community based health insurance scheme, to include a range of advanced and specialized medical services This move is designed to improve access to care that was once too costly or only available abroad.
Among the newly integrated services, Rwanda has significantly increased its capacity to treat kidney failure and heart conditions locally Since the launch of the domestic kidney transplant program in 2023, approximately 88 kidney transplants were performed in 2025 alone, marking a dramatic rise in local treatment access and easing the financial burden on patients and families who previously had to travel overseas for care.
Cardiac care has also made remarkable strides In 2025, Rwandan health facilities carried out 861 heart surgeries, including procedures for both children and adults, an achievement that underscores the growing strength of local expertise and infrastructure.
Before these expansions, only a small number of transplant and cardiac procedures were available locally For example, earlier years recorded about 44 kidney transplants and 541 heart surgeries conducted in Rwanda through government programs.
The insurance expansion goes beyond surgical care to cover cancer treatment services, including chemotherapy, radiotherapy, and associated medications, as part of a package of 14 advanced services fully integrated into Mutuelle de Santé by mid‑2025.
The range of services now covered under Mutuelle includes kidney transplants providing life saving treatment previously unavailable locally heart surgeries including paediatric and adult procedures cancer care services such as chemotherapy and radiotherapy dialysis services vital for patients with chronic kidney disease with costs significantly reduced in local facilities interventional cardiology and minimally invasive surgeries improving recovery times and reducing complications and orthopedic procedures including hip and knee replacements.
To support these expanded services while maintaining broad participation, Mutuelle de Santé contributions were updated across socio‑economic categories.
For the first division, the annual contribution reaches 4,000 FRW per person fully covered by the Government For the second division, citizens pay 3,000 FRW while the Government subsidizes 1,000 FRW For the third division, the contribution is 5,000 FRW per person per year For the fourth division, the annual contribution is 8,000 FRW and for the fifth division, it rises to 20,000 FRW per person per year.
These structured contribution levels ensure that lower income groups continue to receive support while higher contributors help sustain the insurance pool as costly services are added.
Experts note that this expansion significantly reduces the need for foreign referrals Between 2020 and 2025, Rwanda referred 352 patients abroad at a cost exceeding $6 million, mainly for cancer, heart, and kidney treatments, a trend now in decline thanks to growing local capacity.
